Apple's iOS 5 operating system introduced a feature called "Multitasking Gestures", an enhancement that allows you to switch between apps using just a multitouch gesture. Unfortunately, this will accidentally "take over" touch input for apps when you have your wrist on the glass, and Penultimate cannot automatically correct for it. Please disable Multitasking Gestures in the iPad Settings app:
